Position Description

Case Manager - Licensed Practical Nurse (LPN)

Hours: M-F 8am to 5pm

Contract (ongoing)

Position Overview: The Case Manager LPN is responsible for ensuring the seamless coordination of care for patients across inpatient and outpatient settings. This role involves working closely with patients, families, caregivers, and healthcare teams to optimize treatment plans and improve health outcomes within the parameters of established contracts and patients’ health plan benefits.

Key Responsibilities:

  1. Ensure continuity of care by utilizing appropriate resources within the inpatient and outpatient settings.
  2. Perform care management functions either on-site or telephonically as required.
  3. Collaborate with the care team and Primary Care Physician (PCP) to coordinate patient care.
  4. Meet with patients, families, and caregivers to discuss and implement treatment plans in accordance with the Transitions of Care program.
  5. Identify and facilitate follow-up care for high-risk patients in acute care settings, skilled nursing facilities, custodial, and ambulatory environments.
  6. Consult with physicians and care team members to ensure successful implementation of patient care plans.
  7. Coordinate treatment plans with healthcare professionals and triage interventions based on team member expertise.

Qualifications & Experience:

  • Licensure: Current and valid Florida LPN license.
  • Certification: IV Certification required.
  • Experience:
  • Minimum of 1+ years in an acute clinical care setting.
  • Previous experience in care management, utilization review, or discharge planning.
  • Familiarity with HMO processes and guidelines.

Preferred Skills:

  • Strong communication and interpersonal skills for patient and family engagement.
  • Ability to coordinate care across various medical settings.
  • Proficiency in electronic medical records (EMR) systems and care documentation.
  • Problem-solving skills with a patient-centered approach.
